Volunteer by joining or establishing a NMDP chapter on your campus, leveraging the power of your school’s collegiate athletics by hosting a Get in the Game event or being a Legislative Advocate. NMDP VOLUNTEER CONFIDENTIALITY STATEMENT

Volunteers are required to keep confidential all donor, patient, employee or volunteer information, or NMDP privileged or proprietary information encountered while volunteering for NMDP. Volunteers may use such confidential information only in a manner consistent with and necessary for the completion of their defined volunteer duties. Volunteers are strictly prohibited from disclosing this or other confidential information to any person who does not have a legitimate business or legal need to know.

All volunteers must adhere to this confidentiality agreement. Failure to maintain confidentiality may result in termination of the volunteer’s relationship with NMDP or other corrective action, including the possibility of legal action.

I understand and agree that my photograph or video images may be made and/or used, in any form, in memorializing, promoting, or advertising NMDP, and that no compensation will be due to me in that regard. I irrevocably grant NMDP Foundation the exclusive right and authority to copyright, use, and publish such images for these purposes.
I recognize and freely acknowledge the risks and dangers of volunteering and fully assume all such risks and dangers arising from that participation, without limitation including accidents, physical injuries or death, property damage, any results of my negligence or the negligence of other participants or the Sponsors, any results of failures in my or others’ equipment, possible physical and/or mental exhaustion, possible COVID-19 exposure or infection, and all the costs and expenses I incur with respect to these risks and dangers. Neither I nor my beneficiaries, heirs, next of kin, spouses, or assigns will sue or make any claims against the Sponsors in connection with My Claims or the risks and dangers I have assumed. I recognize and freely acknowledge that volunteering requires adequate physical and mental conditioning, and I represent that I am in sound physical and mental condition, sufficient to participate in the Event. I have no physical or mental impairment that would endanger others or me.
I agree to indemnify and hold harmless NMDP Foundation and NMDP, its officers, directors, employees, agents, assigns and insurers from all claims, liability, and damages I may sustain from any cause, including negligence, as a result of my participation as a volunteer at this opportunity.

All minor volunteers must have a parent or guardian sign the waiver and confidentiality statement above on your behalf. If you are under the age of 18, ask your parent / guardian to complete the exemption section (confirm consent for exemption, name and email electronic) before finalizing the registration.
